821 and 921 GC Services
Working with delivery hospital closely is expected as professionals of Yulane’s delivery and afterbirth arrangements. Ensure that you are aware that SSN and Birth Info to Vital Records Database will be provided by the delivery hospitals. This services is being occurred at the same day of the birth of child is given. 821 process is the GC arrangement and delivery services. 921 processes is the afterbirth arrangement. These services can be provided by the Birth Attorney ( can be the escrow attorney ) or IP authorized POA professionals per the experiences with the following standard procedures .
- Medical Account Setup $400 + $80 for each bill: This service can be provided by either GC attorney or escrow attorney. The Medical Account Setup is the Yulane’s stand template that states the matching information of co-parents. Each bills from the GC’s hospital or infant care providers to IP will be paid $80.00. The completions of this services is to have signed statement of total amount owed and paid per the monthly basis. In the event, the escrow attorney could not collect enough information per month, it is accepted by the end of the closing GC case, the document is expected to be received within 60 days from the day of birth of child.
- $700 Arrangement of Delivery Hospital: in the event GC is not from the IL and agreed to deliver the babies in a hospital. The services mainly communicate with both IL hospital and GC to reach out the terms signed at 634 Compensation that is part of GCS. The completion of this services is to have the GC agreement signed with Delivery Hospital and Fees are collected and deposit with Credit Card at Delivery Hospital as the result of this services.
- $400 IL Parentage Document Submission: the IL GC attorney will help arrange the Parentage Document titled SURROGATE PARENTAGE STATEMENT INTENDED MOTHER/CO-PARENT Submission per IL Surrogacy Statutes. The completion of this services is to let both IP and GC complete the signatures on the required documents working with delivery hospitals. Generally the delivering hospital will have its own documents and will submit the signed SURROGATE PARENTAGE STATEMENT INTENDED MOTHER/CO-PARENT to Illinois Department of Public Health Division of Vital Records, The service includes the 2 copies of the birth certificates received in the office of Public Health Division of Vital Records.
- $1500 Arrangement of Hiring Birth Attorney of Other States: This services is to hire legal professionals from other states to purse the parentage rights accounting to the laws of the state where the GC will deliver the newborn. In the event the law is not in favor of surrogacy, the birth attorney is required to file the petitions to the parentage document in that birth-state. The birth-state attorney generally is paid $1500 to $2500 plus expenses as the standard fees in the event the IP’s parentage paperwork is given by the court. The completion of this services is to have certificate being received by IPs by Affidavit of Newborn Certificate signed by by GC attorney
- POA and Breastfeeding Documents: Other arrangement documents such as POA for IPs, Breast feeding arrangements is/(are) about $450 each using the Yulane’s templates.
- Travel Expense : In the event travel services are required for both 821 and 921 services, a hourly pay $50.00 will be paid by IPs that include the local travel , tolls, gas and parking etc. enclosed. Dinner with IP is not paid. Anyhow the out-of-state travel will be paid with hotel up per diem to $200 per night that include the hotel, meal, gas etc. plus a rate $400 a flat fee per night as professional services. It is $600 per day.
We do about 20 deliveries a year in IL since 2010.
