SAP Prescreening Procedures

First, we want to thank each of you again for supporting Yulane. In addition, we also want to make special thanks and many thanks to those surrogate ladies whose efforts are truly amazing gifts that have made all these beautiful events happen and happening. Your contributions are milestones. We want to thank you again for helping the globe Chinese internet parents. Without you, there would be no Yulane’s fertility programs that help our childless families who have conceived babies and achieved the dream of having a true family.
Yulane SAP 2013 Program continues. In the respect of ASRM recommendation and guidelines, the surrogate ladies should have no more than five previous vaginal deliveries or two previous cesarean deliveries. The Yulane SAP 2013 program would need the agency's assistance to comply with the ASRM recommendation.
Yulane Surrogate Program guidelines on who can be a gestational carrier. The Program follows the American Society of Reproductive Medicine (ASRM) 2012 Practice Committee recommendation on screening potential surrogates.
Any agency involved in surrogacy should screen their potential candidates by history to assure compliance by these ASRM recommendations before referring them for further medical evaluation.
In summary:
Who can be a gestational carrier (GC)?
A healthy woman between the ages of 21 and 45 who has had a successful term pregnancy and has a supportive family environment to help her cope with the added stress of pregnancy.
The GC should have no more than five previous vaginal deliveries or two previous cesarean deliveries. Prior to becoming pregnant, the GC should consulate about the risks of pregnancy with her healthcare provider.
What type of medical tests does a gestational carrier need to have?
The GC should have a complete history and physical examination performed to ensure that there are no reasons for her to avoid pregnancy. This examination also will identify patients at risk for sexually-transmitted diseases.
The American Society for Reproductive Medicine (ASRM) recommends that all carriers be tested for viral infections, including HIV, hepatitis, gonorrhea, chlamydia, syphilis, and cytomegalovirus.
Does the gestational carrier have to undergo psychological testing?
A GC and her partner (if appropriate) should have a clinical interview with a mental health professional. This and subsequent interviews will cover the potential psychological risks associated with the process including managing relationships with her partner, her children, her employer, and the intended parents. Psychological testing may be performed at the discretion of the counselor.
Yulane SAP Program will work with you to improve the screening efficiency. We could get them matched within a 7-business days once approved by IHR., the primary IVF clinic of Yulane SAP that work with Yulane provided 87% of pregnancy rate in year 2012 . If approved by other clinic, the matching process could take longer.
SAP Prescreening Procedures
The following procedures is voted by SAP legal professionals and clinic set forth as SAP Prescreening Procedures version one (1) .
Once your profile and required documents including OB records sent to Yulane SAP division, within 3-7 business days, a SAP (Surrogate Assessment Program) interview manager who generally either is your agency's case manager
or a SAP interview professionals will call to go over the SAP screening process with you over the phone with the derailed explanation on how the SAP screening process will be held. After the phone interview, with the information provided over the phone, the Yulane SAP 634 Gestational Carrier Application ( 634 Form) will be sent to you for signing. Once we received the signed 634 Form from you, you are accepted to enter the SAP Screening Program. Followed is the Travel Arrangement for having you officially come to Chicago for the 1059 SAP Screening.
1059 SAP Travel Schedule is required to be arranged in advance of two weeks prior to the actual travel time with the confirmation from Yulane 's fertility clinics that internet parents are working with. Once your OB records are approved, Yulane RGI will have you come to Chicago for a physical exam, including; blood tests and a Saline Infusion Ultrasound (SIS). If it is possible to arrange a same day return trip traveling from your home to Chicago this is preferred and will help expedite the matching process. Travel will be made by your Agency (actual travel costs), and $150 per diem per day, for meals & transportation if more than one (1) day is required due to travel limitations; i.e. available flights. Surrogate must fly home the same day if medical appointment and air travel allow.
SAP Screening DAY will have two major events in one day. One is medical exams and another is general interview. If there are more time, our SAP team members generally will take you for a dinner at a nice place chatting with you for understanding more about Yulane's Indented Parents' stores and backgrounds.
