Surrogate Requirements

SURROGATE REQUIREMENTS 

In order to qualify as a surrogate mother, you need to be:
(1) healthy, between 21 and 35 years of age
(2) have had one or more children through easy, problem-free pregnancy and delivery
(3) live a healthy lifestyle
(4) have stable home environment
(5) drug free and non-smoker
(6) good mental health
(7) good support system
(8) complete all applications and schedule all appointments promptly
(9) pass criminal and background checks
(10) medical insurance preferred

(11) Comfortable taking medications to prepare the uterus for implantation and to help maintain the pregnancy.  Medications may be in the form of a pill, a patch, a vaginal suppository, or an injection

(12) understand as a surrogate candidate that both myself and my husband/partner will meet with a licensed mental health professional for psychological evaluation

(13) will consent to a full medical examination, including disease testing and drug testing

(14) understand you must act responsibly, and understand that the intended parent’s will depend on you to take the medications as directed by a physician, to attend all doctor appointments, and to take care of your physically and emotionally

(15) Agree for provide the access right and release the protected health information. Yulane SAP program has full time attorney to review the surrogate applicant’s health information before the trip to Chicago, all recorded will be sent to Yulane RGI directly.

(16) An abortion is medically referred to as a therapeutic abortion when it is performed to save the life of the pregnant woman; prevent harm to the woman's physical or mental health; terminate a pregnancy where indications are that the child will have a significantly increased chance of premature morbidity or mortality or be otherwise disabled; or to selectively reduce the number of fetuses to lessen health risks associated with multiple pregnancy. As you might know pre-implantation genetic diagnosis (PGD) is used to screen for genetic disease, it avoids selective pregnancy termination as PGD makes it highly likely that the baby will be free of genetic defects. Most Yulane IPs are required to have PGD performed as part of our program. Should PGD fail to successfully identify a genetic defect or if one of the other potential complications identified in part (1) are identified during pregnancy, we would like to share with you that a therapeutic abortion is required. If one of these conditions is identified by doctors during pregnancy, a therapeutic abortion is referred.   


The qualifying process includes interviews and evaluation by our staff, physician, and a psychologist. You will also be subject to drug tests and infectious disease screening.
