  1. We are writing to you especially requesting PGD screening for sex selection in conjunction with 5-chromosomes by FISH testing panel described by RGI for embryos generated from the requested donor retrieval  cycle(s). This request will be confirmed fully in the separate Embryo Transfer Authorization. 我们特别强调，对每次取卵的所形成的胚胎，需要做RGI实验室描述的PGD的伴性诊断（性别）筛查服务，每次对移植胚胎数量和胚胎性别的需求会我们会单独授权. 

  2. In the event that Donor’s monitoring report shows less than 15 follicles on the 7th day of the period (or on the period day-7), the default decision for Egg Donor SEVEN  (7)  DAY Cycle Stimulation is to cancel the cycle unless IVF physician agrees to provide a better treatment with the permission of Yulane LAB and/or IP to continue.  An email permission letter is authorized to be sent to Yulane Laboratories at jianchi@wecare.yulane.com and susan@wecare..yulane.com.  Dr. JianChi Ding  will make the immediate communication with  Yulane Lab or IP for confirmation of either cycle cancellation or permission to continue the cycle stimulation and then send the feedback to IVF clinic(s).   我们特别强调，对本位捐卵天使，我们要求诊所在捐卵天使例假的第7天做出评估。如果《捐卵天使促排卵检测报告》显示捐卵天使卵泡数量少于15个，一般情况下，医生应该停止给捐卵天使继续促排。 我们要求医生通过玉兰实验室征求我们的同意后，决定是否可以继续促排卵。如果医生认为可以调整方案继续促排，医生需要把促排报告和调整后的方案及理由送给玉兰实验室的jianchi@wecare.yulane.com and susan@wecare..yulane.com。Jianchi Ding 应即时与玉兰实验室和预期的父母讨论并确认是否可以继续进行促排卵。我们授权玉兰实验室的drhuang@wecare.yulane.com  and susan@wecare..yulane.com以邮件的形式和诊所沟通我们对是否需要继续进入促排卵周期的决定。 

  3. In the event the Donor’s monitoring report show greater than  15 Yulane  7-DAY Follicles ( See Definition as above)  on the 7th period days,  the default donor stimulation process is to continue the cycle stimulation and  allow the donor officially enter the Donor Retrieval Program.  An email nonfiction is required to be sent to Jianchi@wecare.yulane.com , susan@wecare.yulane.co. 如果《捐卵天使7天促排卵检测报告》显示捐卵天使卵泡数量高于15，医生或诊所可以不征求我们的同意, 根据诊所IVF取卵标准，继续促排卵，然后完成取卵周期。

  4. We specifically are requesting the IVF clinic provide us the “Donor Fertility Report” and " Intended Parents PGD Report" as part of Egg Donor Egg Retrieval Program.  我们要求IVF/胚胎移植诊所，提供《捐卵天使取卵和胚胎生成报告》和《贵宾PGD胚胎报告》，将取出的卵子数量，受精卵数量和PGD的结果通过玉兰实验室送达给我们。
