- Supporting Document Affidavit of Support is part of the Yulane’s legal services for serving the purpose of lawful statement made by the clients. Collecting correct information generally is provided by the case manager, or authorized 2rd part or the client her/himself. The collected information may be part of the Affidavit of Support can be the core value of the supported matters. The template is to help the IVF professionals including attorney, case managers or authorized parties understand the contents of each document. - Editable PDF Application Installed at your workplace. All supporting documents are provided by the IVF professionals including attorney, case managers, authorized parties or client her/him that will be as part of the Affidavit of Support. Once the information is collected, the authorized parties or client her/him will send to the case managers, or attorneys who are required to have the Editable PDF applications installed on their official able to apply the Yulane templates.
- Fee: $150 for each page and $50 for each additional page on Affidavit document only. Supporting document(s) provided by third party is(are) not part of affidavit statement.
- Additional Affidavit Services: Any additional affidavit services are required to have fee proposal sent to the case managers who will share with IPs. The additional charge for unauthorized affidavit services are generally not allowed unless there is fee agreement authorized by IP or IP’s authorized party.
